If you have a new patient or consultation appointment booked at the practice, we require you to complete the patient detail and medical history forms prior to your arrival.

Cancellation Policy:

Cancellations at short notice and missed appointments have a great effect on the performance of the practice and as such we would wish to avoid them. Obviously, emergencies do occur, and we will be completely understanding of this.  However, appointments missed, cancelled, or postponed, without 48 hours’ notice and without sufficient reason will incur a charge This will be proportionate to the treatment missed. All missed appointment charges will need to be settled in full before more appointments can be made.
